1. Fill out the Online Enrollment Form.

2. Scanned copy or photo of Previous Report Card/Grading Card (For new and transfer student)

3. Scanned copy or Photo of Philippine Statistics Office (PSA) issued Birth Certificate (For new and transfer student)

4. Proof of Down payment and Books/Module Fee payment.

Onsite/In-Person

1. Enrollment Form

2. Original and Photocopy of Philippine Statistics Office (PSA) issued Birth Certificate. (For new and transfer student)

3. Original and Photocopy of Previous Report Card/Grading card. (For new and transfer student)

4. Down payment and Books/Modules fee payment.

*Book/Module payment is on cash basis and must be paid 1 month before the commencement of class.

1. Register online by visiting this link

(A non-refundable reservation fee /down payment is payable on submission of your application. Please ensure you have read the Enrolment Policy before completing your application.)

2. Fill out the application form. Use a Gmail account to register. Make sure the spelling of the email address is correct and that all information is updated, accurate, and reliable.  

3. Upload your previous report card/grading card and a digital copy of your PSA Birth Certificate. (For new and transfer student)

4. Proof of downpayment and book/module fee payment, forward to either of the following:
